What to bring with you:

1. Your picture ID or driver's license

2. All insurance cards

3. Order from your physician for services to be performed

4. List of current medications

5. Anticipate a call from our scheduling department to collect your required estimated financial responsibility. Any unpaid co-payments or deductibles are expected to be paid at the time of service.

6. If you need to make financial arrangements, please contact our financial counselor prior to your date of service to avoid any delays on the day of service.
