The Home Health staff is comprised of Registered Nurses, Licensed Practical Nurses, Physical Therapists, Occupational Therapists, Home Health Aides, Speech Therapists, and Licensed Clinical Social Worker. DeSoto Memorial Home Health Care can provide health related services whether a patient is being discharged from the hospital, a rehabilitation facility, nursing home, or currently in their own home and in need of professional services as ordered by their provider.

The ultimate goal for our patients is to ensure they become self reliant. The Home Health staff will consistently reinforce patient and family education on each and every visit.
