Registration

Outpatient Registration Services is open Monday through Friday from 6:30am -5pm. Saturday hours are 7:30-11:30. Our direct line is 494-8459 In order to expedite your registration process, please bring your physician order, this order must be signed and have a diagnosis and the test on each prescription.

Diagnostic Imaging Scheduling is open Monday through Friday from 8:00am-5pm. Our direct line is 491-4390. When scheduling your appointment please provide the following information: The name of the exam being performed, a diagnosis and your insurance. Some insurance companies will require an authorization prior to having the exam performed.

Most insurance carriers are accepted; please contact a financial counselor for additional information.

For our self-pay patients, we offer a 30% discount.
