DeSoto Memorial Home Health Care

In 1992, DeSoto Memorial Hospital began its Home Health Care Agency.  First located within the hospital, then relocated to an off-site location and finally nestled back onto the hospital property in a stand alone building shared with Physical Therapy.  The Home Health staff is comprised of Registered Nurses, Physical Therapists, Occupational Therapists, Licensed Practical Nurses, Home Health Aides, Speech Therapy and a Licensed Clinical Social Worker.  DeSoto Memorial Home Health Care can provide health related services whether a patient is being discharged from the hospital, rehabilitation facility, nursing home or currently in their own home and in need of professional services as advised by their physician.  

The ultimate goal for our patients is to ensure that they are self – reliant.  The Home Health staff will consistently reinforce patient and family education on each and every visit.  Competence and independence in healthcare is shifted to the patient and/or caregiver as soon as reasonably possible.  Our staff prides themselves in providing detailed, compassionate and interdisciplinary care that involves collaboration with the physician.  DeSoto Memorial Home Health Care is ready to assist patients with health care services.  In addition to obtaining a doctors order, there are other requirements to initiate Home Health Services.
